Emerging Mpox Threat: A New Recombinant Strain and What You Need to Know
Mpox, formerly known as monkeypox, is undergoing a concerning evolution. While historically contained within specific regions of africa and primarily spread from animals to humans, we’re now facing a new phase of the outbreak – one characterized by increased person-to-person transmission and the emergence of a novel, recombinant viral strain.this article breaks down the latest developments, explains the risks, and outlines how to protect yourself.
A Shifting Landscape: From Zoonotic to Human-to-Human Spread
For years, mpox cases in the Democratic Republic of Congo (DRC) were largely linked to direct contact with infected animals, typically through handling bushmeat. However, a recent outbreak originating in the DRC in 2023 has dramatically shifted the dynamics. We’re now seeing sustained human-to-human transmission, notably within families and healthcare settings lacking adequate protective equipment. This is especially impacting children under 15 in affected African nations.
Importantly, this outbreak is also demonstrating a new transmission route: sexual contact. While historically rare, sexual transmission – predominantly heterosexual – is becoming increasingly common, with observed clusters among sex workers and transient populations.
Global Spread and the Rise of Clade Ib
Between January and November 2025, over 48,000 cases of Clade Ib mpox were reported across Central and Eastern Africa. While the majority of cases remain concentrated in these regions, approximately 43 cases have been identified in other countries. Notably, around half of these international cases involved recent travel to Africa, but a significant 24 occurred in individuals with no travel history, indicating local spread.
The World Health organization (WHO) now confirms local person-to-person transmission of Clade Ib mpox in Italy,Malaysia, the Netherlands,Portugal,Spain, and the United States. This underscores the virus’s growing ability to establish itself outside of its traditional endemic zones.
A New Hybrid: The recombinant Virus
Recent genomic sequencing in England has revealed a particularly worrying development: a recombinant mpox virus. This new strain combines genetic material from both Clade IIb and Clade Ib, essentially creating a hybrid. while viral recombination is a natural evolutionary process, it raises concerns about increased transmissibility and potential changes in disease severity.
As Dr. Boghuma Titanji of Emory University aptly stated, “The more mpox circulation we permit, the more opportunities the virus has to recombine and adapt, further entrenching mpox virus as a human pathogen that is not going away.” This highlights the critical need for proactive public health measures.
Protecting Yourself: The Power of Vaccination
Fortunately, a safe and effective vaccine exists: JYNNEOS. This vaccine provides protection against both mpox and smallpox, administered in two doses, four weeks apart. Vaccination substantially reduces the risk of infection, limits disease severity, and decreases the likelihood of transmission.
Currently, the CDC recommends JYNNEOS vaccination for:
* gay, bisexual, transgender, and other men who have sex with men who have had a sexually transmitted infection in the last six months.
* Individuals with multiple sexual partners.
* Those engaging in anonymous sexual or intimate contact.
* People who frequent commercial sex venues.
* Individuals participating in sexual activity associated with large public events in areas with mpox transmission.
This targeted vaccination strategy proved accomplished in curbing the 2022 mpox outbreak in the United States. If you meet these criteria and haven’t been vaccinated, prioritize getting vaccinated as soon as possible. While the recombinant strain hasn’t yet been detected in the US, both Clade IIb and Clade Ib continue to circulate at low levels.
staying Informed and Vigilant
The evolving mpox situation demands continued vigilance and proactive public health responses. Staying informed about local transmission patterns, practicing safe sexual behaviors, and prioritizing vaccination are crucial steps in protecting yourself and your community. We will continue to monitor the situation closely and provide updates as they become available.
