Their recent study, published in the Journal of neurochemistry, utilized a mouse model and revealed compelling results. Female mice with the APOE4 gene demonstrated significantly healthier gut bacteria and increased brain energy levels when following a ketogenic diet compared to those consuming a high-carbohydrate diet. Interestingly, this benefit wasn’t observed in male mice, highlighting the potential for sex-specific responses to dietary interventions.
Why Ketones Matter: The Brain’s Alternative Fuel source
The key to understanding this phenomenon lies in how the brain utilizes energy. Traditionally, the brain relies on glucose, derived from carbohydrates, as its primary fuel source. However, for individuals with the APOE4 gene – particularly women – this glucose conversion process can be inefficient, potentially leading to cognitive decline over time.
“When we eat carbs,our brains convert the glucose into fuel,” explains Ivanich. “But those with the APOE4 gene struggle with this conversion, impacting brain energy. By switching to a keto diet, the body produces ketones, which serve as an alternative fuel source. This may preserve brain cell health and decrease the risk of developing Alzheimer’s.”
Essentially, the ketogenic diet bypasses a metabolic bottleneck, providing the brain with a readily available and efficient energy source.

For Ivanich, this work is deeply personal. “My grandmother’s experiance with Alzheimer’s sparked my interest in this field,” she shares. “Being able to contribute to research that could help others preserve their brain health is incredibly rewarding.”
