Isatuximab Subcutaneous Delivery: Body Weight Considerations in Multiple Myeloma Treatment
As a hematologist,I’m always focused on optimizing treatment for my patients with multiple myeloma.Recent data from a phase 3 study offers valuable insights into the use of isatuximab delivered via on-body injector (OBI) versus intravenous (IV) infusion, particularly concerning how body weight impacts outcomes. Understanding these nuances allows for more personalized and effective care.
Consistent Efficacy, with a Notable Difference
Generally, the study demonstrated that isatuximab, whether administered subcutaneously with the OBI or intravenously, maintains consistent efficacy across a range of body weights. However, one interesting observation emerged: patients receiving OBI isatuximab experienced a statistically meaningful improvement in overall response rate (ORR) – a 5% increase – compared to those receiving IV isatuximab, and this benefit was specifically seen in patients with higher body weights.
Discontinuation rates and Neutropenia
Treatment discontinuation rates were relatively low for both management methods, varying slightly based on weight categories. Specifically, discontinuation occured in approximately 14% of patients on OBI and 6% on IV (for those ≤65 kg). These rates shifted to 8% and 10% (for patients >65-≤85 kg) and 2% and 10% (for those >85 kg).
It’s important to be aware of potential side effects. A similar incidence of grade 3 or higher laboratory neutropenia – a decrease in neutrophils, a type of white blood cell – was observed across moast weight groups with OBI isatuximab. However, patients weighing 50 kg or less experienced a notably higher rate of this side effect with OBI (93.8%) compared to IV (71.4%).
Pharmacokinetics and Flat Dosing
Fortunately, the analysis revealed no direct correlation between the amount of isatuximab in the body (pharmacokinetics) and the occurrence of side effects, nonetheless of whether it was given via OBI or IV. This is reassuring, as it suggests a consistent safety profile.
Notably, a “flat-dose” approach with the OBI - meaning the same dose is given regardless of weight – proved effective. This simplifies treatment administration and avoids complex weight-based calculations. The observed differences in outcomes appear to be more closely linked to pre-existing disease characteristics rather than body weight itself.
What This means for you
If you are living with relapsed or refractory multiple myeloma and considering isatuximab, these findings are encouraging. The OBI offers a convenient alternative to IV infusion, with comparable efficacy and a manageable safety profile. While the higher neutropenia rate in lower-weight patients warrants careful monitoring, the overall data supports the use of OBI isatuximab across a broad range of body weights.
