Eliminating midlife hypertension, avoiding type-2 diabetes, and quitting smoking can extend a person’s cognitive health span by nearly 13 years, according to a study published in the medical journal Neurology. Researchers analyzed decades of longitudinal data to track how modifiable health factors alter the onset of neurodegenerative disease, offering evidence on the long-term impact of lifestyle choices on brain health.

A team of scientists from New York University drew upon health records from 12’409 participants enrolled in an ongoing American health study launched in 1986. At the baseline of the evaluation, participants averaged 56 years of age and were dementia-free. Over an observation period spanning approximately three decades, roughly 3,000 participants developed dementia, while more than 5,200 individuals died without receiving a dementia diagnosis.

The findings quantify the divergence in cognitive longevity between individuals with and without major vascular and behavioral risk factors. Participants who did not smoke, suffered no high blood pressure, and had no diabetes remained dementia-free for an average of approximately 30 years during the study period. In contrast, individuals who presented all three risk factors lived just over 17 years without a dementia diagnosis. Those burdened with one or two of the conditions averaged 26 and 21 dementia-free years, respectively.

Gender and Demographic Disparities in Cognitive Decline

Beyond the primary risk factors, the data revealed noticeable differences across demographic lines. Independent of how many risk factors they carried, female participants remained dementia-free longer than male participants. Furthermore, male subjects tended to receive a dementia diagnosis at an earlier age, even when accounting for habits like smoking.

Disparities also emerged between racial groups within the cohort. White participants generally lived longer without a dementia diagnosis compared to Black participants. However, the study’s authors noted that these gaps stem in part from structural inequalities within the healthcare system.

The research team emphasized that because these three specific risk factors typically manifest during midlife, they could have lasting consequences on neurodegenerative pathology and overall brain health. Early detection, medical management of blood pressure and diabetes, and proactive smoking cessation are vital strategies for preserving cognitive function late into life.

The findings arrive as public health organizations grapple with a global caseload of cognitive disorders. According to estimates from the World Health Organization (WHO), roughly 57 million people live with dementia worldwide, with that figure climbing steadily.
