“We also appreciate the significant overlap between healthy aging and cardiovascular health, where aging-related inflammation can harden arteries and lead to cardiovascular disease,” explains Dr. Howard Sesso, Division of Preventive Medicine and associate epidemiologist at Brigham and Women’s Hospital, and a founding member of the Mass General Brigham healthcare system. “because of that,we wanted to see whether multi-year cocoa extract supplementation versus a placebo could modulate inflammaging – and the data suggests it does.”
The COSMOS Trial: A Foundation of Evidence
The research builds upon the robust foundation of the COSMOS trial (COcoa Supplement and Multivitamin Outcomes Study), a large-scale, randomized, double-blind, placebo-controlled clinical trial conducted between 2014 and 2020. Involving 21,442 participants over the age of 60, COSMOS previously demonstrated a significant 27% reduction in cardiovascular disease mortality among those taking a daily cocoa extract supplement.
This new study, published recently, sought to understand how cocoa extract might be exerting this protective effect. Researchers analyzed blood samples from 598 COSMOS participants, meticulously measuring key biomarkers of inflammation at baseline, one year, and two years into the study.
Key Findings: Targeting hsCRP and Beyond
The analysis focused on five crucial biomarkers:
* hsCRP (high-sensitivity C-reactive protein): A marker of systemic inflammation.
* IL-6 & TNF-α: Pro-inflammatory proteins.

The results were striking. Participants taking cocoa extract experienced a consistent 8.4% reduction in hsCRP levels each year compared to the placebo group.While other biomarkers remained relatively stable or showed modest increases, the consistent decline in hsCRP is a significant finding.
“The decrease in hsCRP may help explain the cardio-protective effects seen with cocoa extract supplement in the larger COSMOS trial, where participants experienced a reduction in cardiovascular disease death,” researchers noted.
Interestingly, the study also observed an increase in IFN-γ in the cocoa extract group. Senior author Dr. Yanbin Dong, MD/PhD, Director of the Georgia Prevention Institute (GPI) and cardiologist/population geneticist at the Medical College of Georgia/Augusta University, notes this warrants further investigation. “While cocoa extract is not a replacement for a healthy lifestyle, these results are encouraging and highlight its potential role in modulating inflammation as we age.”
