When a donor heart becomes ready, time dictates every subsequent action. Christian Bireta, a transplant surgeon at the Herzzentrum Leipzig, explains that once an organ is retrieved from a deceased donor who opted for donation, a strict countdown begins. “Ab dann schlägt die Uhr rückwärts für vier Stunden,” Bireta told MDR AKTUELL, noting that the transport and surgical implantation must be completed within that four-hour window.

Transport Logistics and the Long Wait

During a typical retrieval, the donor’s heart is cooled directly within the body before a cardioplegic solution halts the circulation. Surgeons then inspect, sterilize, and pack the organ into a specialized transport cooler. While Bireta races back to the clinic—sometimes navigating open highways at high speeds—the receiving patient is prepped simultaneously at the hospital.

Receiving patients often wait up to five years for a suitable match. Upon receiving the notification call, the patient is brought to the facility by ambulance. Even at this stage, the final decision rests with the surgical team. “Ich sage dem Patienten immer: Wir haben ein Organangebot für Sie, aber in letzter Instanz entscheidet der Chirurg vor Ort, ob das Herz gut genug ist oder nicht,” Bireta stated in the MDR AKTUELL report.

Inside the Operating Theater at Herzzentrum Leipzig

Upon arrival at the Herzzentrum Leipzig—where roughly 30 heart transplants are performed each year—the cooler is handed directly to the surgical team working inside the operating theater, where the recipient lies with an opened chest cavity. Throughout the entire donation and transplant process, strict anonymity is maintained between the donor’s family and the recipient. “Das Skurrile ist: Einer stirbt, damit Andere leben,” Bireta noted to MDR AKTUELL, emphasizing that protecting the privacy of both families remains paramount.

The Moment of Reconnection and Relief

The climax of the procedure occurs when the organ is connected to the recipient’s circulatory system. “Wenn ich das Herz bringe, implantiere und die Hauptschlagaderklemme geht auf, das Blut fließt in das Herz und es fängt an, von allein zu schlagen – da fällt mir selbst ein Stein vom Herzen,” Bireta described to MDR AKTUELL.

Post-Operative Vulnerability and Patient Care

Following a successful operation, recovery requires intensive monitoring and lifestyle adjustments. Because immunosuppressive medications are necessary to prevent organ rejection, patients remain highly vulnerable to infections during the recovery phase, requiring strict precautions against everyday pathogens such as fungi found in potting soil.
