Glioblastoma Multiforme (GBM), an aggressive adn frequently enough resistant form of brain cancer, impacts over 300,000 people globally each year. For decades, the standard of care has been the chemotherapy drug temozolomide (TMZ). But its effectiveness is frustratingly limited, largely because tumors develop defenses – specifically, a DNA repair enzyme called MGMT – to counteract the drug’s effects.
However, groundbreaking research is revealing a critical piece of the puzzle that’s been largely overlooked: time of day. It turns out,the body’s natural internal clock,or circadian rhythm,profoundly influences both MGMT activity and tumor sensitivity to treatment. The Shifting Landscape of MGMT & Diagnosis
For years, doctors have assessed MGMT levels - specifically, methylation of the MGMT gene, which effectively “switches it off” – to predict how a patient will respond to TMZ. The logic is simple: less MGMT means the tumor is less able to repair damage caused by the drug, leading to a better response.
But what if those MGMT levels aren’t static? That’s precisely what researchers at Washington University in St. Louis discovered. Analyzing five years of patient biopsy data, they found a clear pattern: biopsies taken in the morning were considerably more likely to show high MGMT methylation.
“Presently,methylation is used for diagnostic purposes in cancer,” explains Dr. Joshua Rubin of WashU Medicine. “This research highlights the possibility that the time of day a biopsy is taken could influence the diagnosis and, consequently, the treatment plan.”
This finding is a game-changer. It suggests that current diagnostic practices may be providing an incomplete - and perhaps misleading – picture of a tumor’s true vulnerability.
TMZ’s Daily Rhythm: Timing is Everything
The team didn’t stop at diagnosis. They investigated whether the timing of treatment could also be optimized.Their hypothesis? If MGMT activity fluctuates throughout the day, so too might the tumor’s sensitivity to TMZ.
And they were right. The research revealed that TMZ is demonstrably more effective at certain times of the day. But when is that optimal window?
“When we found that TMZ is more effective in the morning, we wondered if, perhaps, this coincides with when there is less MGMT around to repair TMZ-induced damage,” explains lead researcher Dr. Herzog.
To pinpoint the ideal timing, the team, including graduate student Maria Gonzalez-Aponte and math biologist Olivia Walch, built a sophisticated mathematical model. This model accounted for the time it takes TMZ to inflict DNA damage and trigger cell death, alongside the daily rhythm of MGMT protein levels.
The Power of Predictive Modeling
“I love when math pushes us past intuition and helps us see something new,” says Walch.And the model delivered. it predicted that administering TMZ instantly after MGMT protein levels peak gives the drug the longest possible window to act while the tumor’s repair mechanisms are at their weakest.
this isn’t a simple equation. Walch emphasizes the complexity: “Figuring out what that right time is can be tough. Dosing time can depend on a multitude of factors, including dose and individual variations in circadian rhythms.” However, the model provides a powerful tool for navigating that complexity and moving beyond guesswork.
Beyond TMZ: A Chronotherapy Revolution?
The implications of this research extend far beyond TMZ. Dr. Herzog is optimistic that this “chronodiagnostic” and “chronotherapy” approach could be applied to other cancer drugs with known circadian targets.
“It may be meaningful to avoid treatment at times of day when it also promotes tumor growth,” Herzog notes, pointing to drugs like dexamethasone, used to manage brain swelling, as potential candidates for time-sensitive dosing.

This research is still evolving, and it will take time to translate these findings into clinical practice. Though, it offers a beacon of hope for individuals battling GBM.
