Higher Triglycerides Linked to Lower Dementia Risk: New Research Challenges Conventional Wisdom
For decades, high triglyceride levels have been viewed primarily as a cardiovascular risk factor. However, groundbreaking research published in the October 25, 2023, issue of Neurology®, the medical journal of the American Academy of Neurology, suggests a surprising and potentially paradigm-shifting connection: higher triglyceride levels in older adults may be associated with a reduced risk of dementia and slower cognitive decline.
This isn’t a call to raise your triglyceride levels. It’s a complex finding that demands a deeper understanding of how these fats interact with brain health,and challenges previously held assumptions about optimal lipid profiles in aging populations.
Understanding Triglycerides and Brain Health
Triglycerides are a type of fat, specifically fatty acids, and represent the most abundant fat in the bloodstream. Crucially, they serve as a primary energy source for the brain – contributing up to 95% of the dietary fats utilized by this vital organ. This energy provision is fundamental to neuronal function, synaptic plasticity, and overall cognitive performance.

The Study: A Deep Dive into the Data
Researchers meticulously analyzed healthcare data from 18,294 participants with an average age of 75, all initially free from Alzheimer’s disease or dementia. Over an average follow-up period of six years, 823 individuals developed dementia.
the team tracked annual measurements of key lipid markers – total cholesterol, triglycerides, LDL cholesterol, and HDL cholesterol – allowing for a nuanced assessment of their relationship to cognitive outcomes. Participants were categorized into four groups based on fasting triglyceride levels,with an average level of 106 milligrams per deciliter (mg/dL) across the entire cohort. Dose-Response Relationship: For every doubling of triglyceride levels,researchers observed an 18% reduction in the risk of developing dementia.

Second Group (63-106 mg/dL): 5% developed dementia (15% lower risk compared to the lowest group).

Validation and Cognitive Function
To strengthen the findings, the researchers replicated the study using a separate dataset of 68,200 older adults from the U.K. Over an average of 12 years,2,778 individuals developed dementia. This validation cohort mirrored the initial results, demonstrating a consistent 17% decreased risk of dementia with each doubling of triglyceride levels.

Nutritional Status: Adequate fat intake, including triglycerides, is essential for brain health. Lower levels might indicate malnutrition or restrictive diets.
