After 27 years of living as a woman, a shocking examination of a woman in China revealed the presence of testicles in her stomach, meaning that, biologically, she is a man.

SCMP reported that Li Yuan, from Hubei province in central China, whose name was changed to protect her identity, was worried about the absence of menstruation and delayed breast development since puberty.

She went to a local hospital when she was 18 and was diagnosed with abnormal hormone levels and potential ovarian failure. Doctors also recommended further chromosome testing at that time, but Li and his family did not take the suggestion seriously.

Recently, when Li was planning her wedding, she decided to undergo a thorough checkup.

This time, Duan Jie, a gynecologist, diagnosed a rare disorder, congenital adrenal hyperplasia (CAH). After waiting anxiously for the test results for a month, the doctor’s decision was confirmed.

Li has male sex chromosomes, but looks female.

“Socially, Li is a woman. But chromosomally, she is a man,” explained Duan.

Congenital adrenal hyperplasia affects a child’s normal growth and development. Symptoms in babies may include ambiguous genitalia in girls and an enlarged penis in boys.

Li was shocked by the news. Living as a woman for 27 years, she struggled to accept the truth. Only around 1 in 50,000 newborns suffer from this form of CAH.

Both of Li’s parents carry the recessive gene that causes the disease, so Li has a one in four chance of passing the disorder to him. Due to lack of early treatment, Li also suffered from osteoporosis and vitamin D deficiency, according to test results.

The doctor recommended immediate surgery to remove the testicles hidden in Li’s stomach because of the high risk of cancer. In early April, Li successfully underwent a dangerous operation and had his organs removed.
