The Efficiency-reimbursement Paradox
Home health agencies are increasingly turning to technology to optimize operations. You can significantly amplify your team’s capabilities with the right tools, but even these gains are being offset by significant reimbursement pressures.
Steven Gonzalez, of HealthView, highlighted this reality.”You can 10x an employee by amplifying them with technology, but the reality is, we did have to lay people off, just like a lot of other home health [companies] had to do.” This isn’t an isolated incident. Agencies across the country are facing similar difficult decisions. gonzalez shared that these concerns were directly addressed with members of Congress and the California Senate last week.
Understanding the Reimbursement Cuts & The Alliance’s Response
The core of the problem lies with proposed Medicare payment cuts. Hillary Loeffler, VP of Policy and regulatory Affairs at the National Alliance for Care at Home (the Alliance), explains that the current methodology used to calculate these reductions is flawed.
“We don’t believe that they’re calculating the reductions appropriately and following the intent of the statute,” Loeffler stated. If finalized as proposed, the impact would be devastating.
The Home health Stabilization Act of 2025: A Potential Lifeline
The Alliance is actively advocating for a solution,and a key piece of legislation offers a potential reprieve: the Home Health Stabilization Act of 2025. Introduced in September by Representatives Kevin Hern (R-Okla.) and Terri Sewell (D-Ala.), this bill proposes a two-year pause on the scheduled reimbursement rate cuts.

* Immediate Relief: Halts cuts for two years, providing breathing room for agencies.
* time for Reform: Allows the industry and lawmakers to collaborate on comprehensive, long-term solutions.
However, Loeffler cautions that even a softened cut isn’t enough. CMS may adjust the proposed rule,but a partial reduction won’t solve the underlying issues.
“I think they’ll probably back off, but to me, success isn’t just cutting it in half or backing off,” Loeffler emphasized. “They really need to revisit the methodology.Right now, the temporary adjustment that they proposed to 5% that’s to start recouping $5 billion from the industry, and unless they change their methodology and how they’re calculating these cuts, which we think they’re making mistakes on, we’re just facing seven, eight, nine more years of 5% cuts to our rates, which is wholly not sustainable for the industry.”

* Be Vocal: The more your voice is heard,the greater the chance of positive change. “The squeaky wheel tends to get the grease, and I think with home health, we need to make our voices heard more loudly and more frequently.”
Beyond Medicare: Addressing Medicaid Reimbursement
The fight for financial stability extends beyond Medicare. Bryan Madden of Bryan Skilled Home Care is actively working on a bill to establish a minimum reimbursement rate for Medicaid managed care. This is particularly crucial for organizations serving vulnerable populations.
