India Approves Qdenga as First Dengue Vaccine for People Aged 4 to 60

Designed for those aged 4 to 60, the two-dose vaccine arrives as health officials report that dengue has shifted from a monsoon-linked seasonal risk to a year-round threat, driven by climate-influenced mosquito behavior.

Regulatory Approval and Vaccine Rollout

Takeda’s Indian unit confirmed the clearance, which follows the vaccine’s authorization in 42 other countries. The drugmaker expects the vaccine to become available in India during the first half of 2027, primarily through private healthcare settings.

Unlike earlier iterations of dengue vaccines, Qdenga is administered as a two-dose, 0.5 ml injection spaced three months apart and does not require pre-vaccination testing for prior exposure. This distinction is critical, as previous vaccines like Sanofi’s Dengvaxia—which is not approved in India—carried risks of severe disease in individuals who had not been previously infected with the virus.

To support this, Takeda has partnered with Indian manufacturer Biological E to bolster production. The collaboration is intended to support an annual output of up to 50 million doses. In late-stage trials, the vaccine demonstrated 80.2% efficacy against confirmed dengue cases one year after the second dose and 90.4% efficacy against hospitalizations at 18 months.

Experts attribute this trend to climate change, which has altered both mosquito life cycles and human water-storage habits. Sunil Kumar Raina, a professor of community medicine, emphasizes that the issue is no longer solely about rainfall but rather about water availability. Because the Aedes aegypti mosquito breeds in stored water—found in coolers, buckets, and containers—it no longer relies on the monsoon to sustain its population. Warmer temperatures accelerate this reproductive cycle, allowing for transmission during months that were historically safe.

Patient Impact and Future Challenges

The year-round nature of the virus has left many patients unprepared. For many, initial symptoms such as fever are often dismissed as common viral infections. The case of Anuradha N., a 52-year-old teacher, highlights the diagnostic challenge: she delayed seeking professional testing because her symptoms occurred outside the anticipated monsoon season, only to later test positive for dengue after her fatigue became debilitating.

While the new vaccine is expected to complement existing vector-control efforts, some experts remain cautious about its ability to curb outbreaks immediately. Divya K.S., an infectious diseases specialist at Apollo Hospitals, noted that while the vaccine is effective at reducing severity and hospitalizations, it is unlikely to prevent outbreaks as long as the mosquito vector continues to circulate.

Looking ahead, other manufacturers are working to navigate the complex regulatory landscape. Hyderabad-based Indian Immunologicals Ltd (IIL) is currently preparing for mid-stage clinical trials for its own candidate. Managing Director K. Anand Kumar explained that the development path remains difficult.
