Invasive lobular carcinoma (ILC) is a distinct subtype of breast cancer that’s gaining increased attention, and for good reason. While frequently enough overshadowed by the more common invasive ductal carcinoma,ILC is on the rise,presenting unique challenges in detection,treatment,and long-term outcomes. What Makes ILC Different?
ILC originates in the milk-producing glands (lobules) of the breast, but its behavior differs significantly from other breast cancer types. Unlike the typical,well-defined tumors of ductal carcinoma,ILC often grows in a more diffuse pattern,infiltrating the breast tissue in single file lines. This growth pattern makes it harder to spot on customary mammograms and can lead to less typical clinical presentations.
Beyond detection, ILC also has a predilection for spreading to different sites then other breast cancers. We’re seeing a higher likelihood of metastasis to the gastrointestinal tract, urinary system, and ovaries. Historically, ILC has been lumped together with ductal carcinoma in research, obscuring these vital distinctions in biology, appearance, and how the cancer responds to treatment.
A Favorable Initial Prognosis, But Long-Term Concerns
While ILC generally presents with a good initial prognosis, it’s not without its complexities. studies show ILC tends to be less responsive to neoadjuvant chemotherapy (chemotherapy given before surgery). Furthermore, women with ILC face an increased risk of developing cancer in both breasts (bilateral disease), experiencing recurrence later in life, and developing a new primary breast cancer in the opposite breast.
Recent data from the American Cancer Society underscores the importance of focused research. As Angela Giaquinto, MSPH, explains, “Even though lobular breast cancer accounts for a little over 10% of all breast cancers, the sheer number of new diagnoses each year makes this disease crucial to understand.” Crucially, survival rates beyond seven years are significantly lower for ILC compared to ductal carcinoma, highlighting the urgent need for improved prevention and early detection strategies.

A recent report utilizing data from the National Cancer Institute and the CDC revealed a concerning trend. In 2021, the incidence of ILC was 14 per 100,000 women, representing 10.6% of all breast cancer diagnoses. And, importantly, these rates are increasing across all racial and ethnic groups.

* Highest Rates: White women (14.7 per 100,000) followed by Black women (11 per 100,000).

* Significant Growth: Asian American/pacific Islander women experienced a 4.4% annual increase.
Survival Rates: A Nuanced picture
The data also reveals a complex picture regarding survival. Women with ILC initially show slightly higher survival rates than those with ductal cancer within the frist seven years after diagnosis. However, this advantage diminishes over time. At 10 years, survival rates are lower for women with regional (78.2% vs. 76.4%) and distant-stage ILC (19.6% vs.12.1%).
Rebecca Siegel, MPH, a senior scientific director at the American cancer Society, emphasizes this point: ”Our study underscores the need for much more facts on lobular cancers…from genetic studies to clinical trial data, so we can improve outcomes for the increasing number of women affected with this cancer.” The unique spread and potential resistance to therapy likely contribute to these lower long-term survival rates.
