Chronic kidney disease is an escalating public health challenge across Germany, where roughly ten percent of adults are estimated to be affected. Yet the overwhelming majority of those living with impaired renal function remain entirely unaware of their condition.

The Freiburg Study Reveals Widespread Silent Pathology

According to a nationwide epidemiological analysis published in the Deutsches Ärzteblatt International, researchers from the Institute for Epidemiology and Prevention at the University Medical Center Freiburg analyzed blood samples from 195,000 adults alongside urine samples from 35,000 participants.

Led by Anna Köttgen and Peggy Sekula, the investigation revealed that nearly one in six participants—18.4 percent—exhibited concerning markers indicative of potential kidney disease when incorporating sensitive urinary assessments. Despite these widespread findings, only five percent of those with abnormal markers reported ever receiving a formal diagnosis.

A Stealthy Decline That Leads to Dialysis

The kidneys function as vital filtration units, producing approximately 180 liters of primary filtrate daily in a healthy adult while reabsorbing all but about two liters. However, this physiological decline often unfolds stealthily and without noticeable symptoms for years.

Sylvia Stracke, a specialist in nephrology at University Medicine Greifswald and spokesperson for the German Society for Nephrology (Deutsche Gesellschaft für Nephrologie, or DGfN), notes that individuals frequently feel completely healthy even when substantial renal function has already been lost.

Symptoms such as nausea, loss of appetite, or cardiovascular complications typically emerge only after significant organ damage has occurred. This frequently leaves patients dependent on lifelong dialysis or organ transplantation. Nationwide, approximately 100,000 individuals rely on regular blood cleansing treatments, while another 20,000 live with a donor kidney.

Routine Diagnostics and the Detection Gap

To evaluate renal health accurately, medical professionals rely on specific diagnostic parameters measured through routine laboratory tests. The Freiburg team determined creatinine levels in blood samples from participants aged 19 to 74. Creatinine is a metabolic breakdown product normally filtered out by the kidneys; when combined with age and sex, this measurement yields the estimated glomerular filtration rate, or eGFR.

Additionally, researchers measured urinary albumin, a protein that typically appears in urine only in minimal quantities. While a single elevated measurement does not constitute a formal diagnosis—since clinical guidelines require abnormal values to persist over at least three months—these diagnostics offer profound insight into hidden disease prevalence.

Missed Opportunities in Primary Care Screening

Experts emphasize that early detection is critical because modern pharmacological interventions can significantly slow or halt the progression of renal damage. Stracke highlights therapeutic options such as renin-angiotensin-aldosterone system inhibitors (RAAS inhibitors), SGLT2 inhibitors that block specific transport proteins within the kidneys, and GLP-1 receptor agonists utilized for blood sugar regulation and weight management.

Despite the availability of these treatments, routine primary care screenings often omit the necessary urine tests due to cost or oversight. Stracke points out that while physicians frequently order the relatively inexpensive blood test, the vital urine test is neglected more often, creating a significant gap in early detection.

Targeted Protocols for High-Risk Demographics

Targeted screening protocols are particularly urgent for older adults and individuals managing conditions such as diabetes, hypertension, obesity, and vascular disease.

Nicole Helmbold, general secretary of the DGfN, recently noted in public commentary that treating chronic kidney disease imposes a massive economic burden on the healthcare system, generating over 24 billion euros in costs during 2020 alone—representing roughly five percent of total health expenditures.

With the World Health Organization estimating that nearly 700 million people suffer from chronic kidney disease globally, and epidemiologist Köttgen estimating approximately 8 million affected individuals in Germany, medical societies continue to advocate for widespread, risk-based screening strategies to catch the condition before advanced organ failure occurs.
