Type 2 Diabetes Cases Rise Among Young Adults and Women in the UK

Type 2 diabetes is increasingly affecting younger populations, with thousands of people under 30 in the UK and rising global concerns over aggressive disease progression. Researchers are now prioritizing the study of youth-onset cases, which often present with more rapid pancreatic failure and higher risks of cardiovascular complications than adult-onset forms.

Rising Diagnoses Among Young Adults

While type 2 diabetes has historically been viewed as a condition of older age, clinical data shows a shifting demographic. According to NHS England estimates, there are now 12,000 people under the age of 30 living with type 2 diabetes in the UK, with women accounting for a larger share of these younger cases. The most common symptoms include feeling very tired, peeing more than usual, and feeling thirsty all the time.

For many, the diagnosis comes during early adulthood. Shannon Ball, 26, was diagnosed with diabetes about 10 years ago while she was in college. She experienced most of the classic ‘four Ts’ symptoms of undiagnosed type 2: going to the toilet a lot, getting thirstier, and feeling more tired more often. After college, I’d fall asleep on the couch, and my nan would find it really hard to wake me up, she recalls. Ball noted that having her grandmother present at that initial appointment helped her navigate the complexity of the information provided by clinicians, as the support allowed her to ask questions about what the diagnosis meant.

Biological Aggressiveness and Puberty Factors

Medical experts emphasize that youth-onset type 2 diabetes is not simply an older person’s disease occurring in a younger body. By the time Kristen Nadeau, MD, MS, began her fellowship in pediatric endocrinology at the CU Anschutz School of Medicine in 2001, concern in her field had risen to the point that the National Institutes of Health launched a major study. As Nadeau and her colleagues began treating more youth with a form of type 2 diabetes never seen in youth before the 1980s, they realized that drugs effective in adults were failing in their pediatric patients, and the disorder was wreaking havoc on their health at unprecedented speed.

Studies have since confirmed the aggressiveness of youth-onset type 2 diabetes, linking it with puberty and finding a significantly higher rate in girls and some ethnic minority groups. Last month, Nadeau and colleagues published a review of that landmark study and two other studies in a special issue of the American Diabetes Association journals Diabetes and Diabetes Care. The review included CU Anschutz experts Phil Zeitler, MD, professor of pediatric-endocrinology at the CU Anschutz School of Medicine, and senior author Dana Dabelea, MD, PhD, associate dean of research and Conrad M. Riley Distinguished Professor of Epidemiology at the Colorado School of Public Health.

According to Nadeau, studies attribute puberty combined with obesity as a likely reason for the disease’s aggressiveness. The pancreas has to make a lot more insulin because of puberty (which fuels increased growth hormone) on top of having obesity, she explained. Youth-onset T2D seems to have more rapid onset of pancreatic failure and the need for insulin treatment, and poorer responses to the medications that were studied than in adults. To address these challenges, researchers are launching a new study called DISCOVERY to dig deeper into the youth version of the disease, hoping to advance a personalized and preventative medicine approach.

Long-Term Cardiovascular and Mortality Risks

The consequences of an early diagnosis are significant. A new study published April 8, 2019, in Circulation, which utilized data from the Swedish National Diabetes Registry to identify 214,278 patients, assessed the excess cardiovascular risk over the lifespan. The research found that individuals diagnosed at age 40 or younger face a substantially higher risk of cardiovascular morbidity and mortality compared to matched controls without diabetes mellitus.

Senior investigator Naveed Sattar, MD, PhD, of the University of Glasgow, explained that the toxicity of type 2 diabetes differs sharply across the age spectrum. When you’re around 20 years old, and diagnosed with type 2 diabetes, you will lose more than a decade of life expectancy, which is on par with type 1 diabetes, Sattar told TCTMD. Conversely, the study showed that there is no increased risk of all-cause or cardiovascular mortality in adults aged 80 years or older diagnosed with the metabolic disorder. Our paper adds more fuel to the idea that being diagnosed with type 2 diabetes in your 20s, 30s, or even 40s is associated with considerably more harm as it relates to the excess risk of complications, Sattar added.
